The Community Connections team are seeking a passionate Care Finder to be part of an exciting and diverse opportunity within IPC Health.

As a Care Finder, you will be responsible for providing a client centred approach that respects and responds to each person’s individual needs, preferences, values, and life experiences. Your focus will be on delivering high-quality care that is tailored to the unique circumstances of each individual. You will engage and build strong relationships with diverse communities, including older persons who are eligible for aged care services and supports, including people from CALD backgrounds, individuals with disabilities, Aboriginal and Torres Strait Islander communities, and those experiencing disadvantage. Your ability to foster meaningful connections with these communities will be essential to your success in the role.

Through assertive outreach and referrals into the service, you will connect with target groups, in collaboration with them identify and assess their health and wellbeing needs, and assist them to create care plans that link them with appropriate aged care services and supports, including My Aged Care. You will also play a key role in establishing and maintaining strong relationships with both internal and external stakeholders to improve referral pathways and enhance the accessibility of aged care services and programs.

The opportunity is fixed term until 30 June 2025 (Special Project) with flexibility to consider Part Time (0.6FTE - 0.8FTE). The role is classified as a Social and Community Services Employee Level 5 offering $97,377 – $101,787 (pro rata if part time) + 11.5% super per annum under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022.

To succeed you will need:

  • Relevant qualifications in aged care, human services, social work, community services or health; or extensive relevant experience and specialist expertise in the aged care sector
  • An excellent understanding of the range of aged care supports and services, access and referral process, including My Aged Care.
  • Understanding of, and demonstrated experience in targeted community and stakeholder engagement strategies
  • Excellent communication, motivational interviewing and holistic wellbeing assessment skills.
  • A sound understanding and knowledge of the Aged Care Sector reforms and potential impacts to clients, service delivery and program operations.
  • A strong understanding of, and commitment to, cultural safety and trauma informed care
