The Opportunity

IPC Health is looking for a highly motivated, energetic and team orientated cardiac rehabilitation nurse to provide cardiac assessment and management services within our new cardiac rehabilitation program in the Brimbank LGA. The role includes individual assessment, therapy, along with coordinating and delivering education presentations primarily on campus, with a mix of groups and individual sessions, consultation may also occur via telehealth. There is an opportunity for some hybrid working (onsite and working from home).

The role will work closely with the current cardiac rehab coordinator, cardiac rehab team and other allied health team members and is primarily based at our Deer Park campus, with occasional attendance at our Hoppers crossing site.

You will make a difference by

  • Provide direct service to cardiac clients and their families, including health education, presentations and therapeutic management.
  • Collaboration with other Allied Health team members to develop, implement and evaluate primary care programs and services in accordance with organisational priorities and expectations.
  • Working closely with referrers and GPs to ensure coordinated service delivery for clients.
  • Participate in program and service planning, quality improvement and review activities as appropriate.

To succeed, you will need

This role is integral in the assessment and rehabilitation journey of clients who following a cardio-respiratory event and or surgery and involves both individual and group interventions.

About You:

  • You are an enthusiastic, dynamic and skilled Registered Nurse who has experience in cardiac nursing care.
  • You are keen to contribute to evidence-based client centred care as part of an experienced, and collaborative interdisciplinary team

What you need:

  • Registration with Nursing and Midwifery Board of Australia via AHPRA
  • Demonstrated 2+ years clinical experience including cardiac nursing care
  • Current Victorian Drivers Licence
  • Demonstration of appropriate competencies and skills in the assessment and management of clients with cardiac conditions.
  • Experience in a rehabilitation/ community setting is highly desirable
  • Post graduate certificate in critical care or equivalent would be advantageous
