Are you passionate about positively impacting the lives of individuals and families in the Fleurieu region? We are seeking an enthusiastic and dedicated Outreach Support Worker to join our highly-driven team.
As an Outreach Support Worker, you will be responsible for delivering a client-centred case management model, assisting the people we work with in securing and maintaining accommodation and support, and providing crisis intervention and ongoing assistance. You will collaborate with the client to develop personalised case plans and empower them to achieve their goals.
Your excellent communication, interpersonal, and negotiation skills will be crucial in supporting the clients' in social and living skills development, promoting informed choices, and fostering independence. With your knowledge of social issues relevant to homelessness and your understanding of complex needs, such as domestic violence and mental health, you will ensure equitable access and equal opportunities.
