ABOUT VILLAGE CONNECT:

Village Connect is a pilot project funded by Department of Communities and Justice to support people 25years or younger, pregnant or parenting, living in SouthWest Sydney. Karitane provide child and family health and wellbeing support alongside partnering organisations including Uniting who provide access to a Key worker and Sonder who provide 24/7 app-based support for health, wellbeing and emergencies. The team includes Health Education officers, Aboriginal staff, Family coach (nurse), Clinical Lead, Administrator and Program Lead, Systems Navigator.

ABOUT YOU:

In this role, you'll provide a range of support for parents and children, from primary to tertiary levels. Key responsibilities include:

  • Building strong connections with children, families, and community stakeholders.
  • Boosting parents’ confidence and capacity, enhancing health and developmental outcomes.
  • Working in various settings, such as offices, community spaces, and family homes.
  • Supporting families with complex psychosocial needs in diverse cultural and socio-economic backgrounds within the SWS community.

ABOUT THE ROLE:

This role aims to improve the wellbeing of children and families, enhancing parents' confidence and competence in their parenting journey. It supports a continuum of care model, offering activities like drop-in groups, individual coaching, formal parenting training, and access to Karitane’s tertiary supports (e.g., PCIT and Residential Unit).

The Family Coach works within a multidisciplinary team, including Health Education Officers, a Parent-Child Interaction Coach, and a broader Village Connect team, collaborating with Key Workers from Uniting and Sonder (an app-based platform). This role is key in building stakeholder relationships, ensuring effective service collaboration, and providing quality, wraparound support to families.

KEY SELECTION CRITERIA

  • Current Registered Nurse (APHRA) with Child & Family Health Nursing post-graduate qualifications, minimum 2 years' experience, and knowledge of the First 2000 Days Framework. Midwifery qualification is a bonus.
  • Relevant qualifications and experience working with families with young children, with a strong understanding of child development and family-centred care.
  • Experience in home visiting or outreach models, providing support in diverse settings such as offices, schools, in groups, community spaces, and family homes.
  • Experience working in multi-disciplinary teams with those with clinical and lived experience expertise.
  • Experience and understanding of supporting families with complex psychosocial needs (including intergenerational trauma) in diverse cultural and low socioeconomic areas of SWS.
  • Experience delivering evidence-based parenting programs and knowledge of Family Partnership principles (or willingness to learn).
  • Strong verbal and written communication skills, with the ability to engage in culturally sensitive community consultations.
  • Ability to work across systems to maintain accurate records and adhere to privacy policies.
