About the opportunity:

Our Housing Support Service aims to achieve long-term housing, employment, education, health and wellbeing outcomes for people who are experiencing homelessness or are at risk of homelessness in the southern metropolitan region.

The case manager role provides support to clients experiencing homelessness or are at risk of homelessness. This is achieved through the provision of a range of intervention options ensuring that the client's needs are identified and tailored goals are developed that promote the client's independence, empowerment, autonomy and dignity. This support may be provided in a range of environments including funded accommodation services, private housing or via assertive outreach to clients experiencing housing crisis.

This role is aligned to the Leading Self proficiency level within the framework, reflecting the responsibility to demonstrate professionalism, personal accountability, self-awareness, and continuous development.

This is a permanent, full-time opportunity, based in Cheltenham.

What you'll be doing:

  • Engage with clients to provide information, education, support and case management services appropriate to their needs.
  • Develop individual case plans in conjunction with clients and implement strategies that enable them to achieve their goals by building upon their strengths and focussing on solutions.
  • Assist clients to develop longer term housing options, both within and/or independently of Launch Housing’s supported accommodation program.
  • Ensure clients maximise all government and community income assistance and support.
  • Initiate and maintain contact with vulnerable clients using flexible engagement strategies to achieve case plan goals.

What we're looking for (key selection criteria):

  • A tertiary qualification in social work, human services or other relevant discipline.
  • Demonstrated understanding of the Victorian homelessness service system especially in regards to crisis and social housing.
  • Experience working with clients with multiple and complex needs due to dual diagnosis, domestic and family violence and/or psychological trauma.
  • Ability to build positive relationships and effectively communicate with a range of stakeholders.
  • Excellent interpersonal and communication skills.
  • Excellent problem-solving and conflict resolution skills.
  • Well-developed organisational and time management skills with the ability to work independently and as part of a team.
  • Ability to use Microsoft Office based applications such as Outlook, Word and Excel.
  • A current Working with Children Check.
  • A valid Victorian Drivers Licence (Mandatory).
