The role

We are looking for a dedicated Community Services Worker (CSW) to join our Metro Beneficiary Programs team – supporting older war widows and people with disability.

You will be required to work from home, conducting visits to clients within the Chatswood, Hornsby & some Western Sydney divisions (Kilometres reimbursed). Ideally, we are looking for a team member located within the Northern Sydney region.

You will also be responsible for our Disability Program – Recreational Activities and Holiday Coordination and must be available to attend 1 Saturday per month in the Bankstown South West region in addition to the contracted hours.

Key Responsibilities

  • Conduct regular home visits to older clients located in the assigned geographical area
  • Advocate for clients by using a holistic person centred approach when making referrals or assisting clients to make referrals for support to maintain their independence
  • Assist in the development and implementation of case plans and review on a regular basis
  • Deliver and coordinate disability inclusion programs in the metro area.
  • Plan and manage community events, including working ten Saturdays annually.
  • Identify and respond to requests for Personal Response Units for clients and refer to appropriate provider (DVA or Legacy). Provide support for installation, decommission and upgrade as required
  • Assist clients (and where applicable their carers) to navigate My Aged Care, NDIS pathways or other service providers
  • Provide quality services to Beneficiaries with Disabilities (BWDs) and their families
  • Provide advice to Legatees to supplement the support they provide clients who may have more complex care needs
  • Develop a sound understanding of key issues for clients to inform service improvement and policy development

Skills and Experience

  • Certificate IV or above in Community Services, Welfare or Disability
  • High level of interpersonal/ communication skills and proven ability to engage in and build trust and rapport with internal/external stakeholders
  • Highly developed organisational skills, with attention to detail and the ability to establish priorities and meet deadlines
  • Proven ability to work independently and as part of a team
  • Ability to travel to visit beneficiaries with use of own private motor vehicle
  • Current NSW Class C driver’s license (unrestricted)

Highly Regarded

  • Sound knowledge of government services such as DVA, My Aged Care and NDIS
  • Current First aid certificate or willing to obtain
  • Proximity to Chatswood, Hornsby, or the Western Suburbs is highly desirable
  • Previous experience with CRM and online case management tools
  • Knowledge of trauma informed practice, person-centred practice and a strength based approach to service delivery
  • Experience with Microsoft Office suite of products including Word, Excel, Outlook and Teams
