Lets Connect Support Services is a small family - owned company delivering Level 2 Support Coordination to NDIS Participants to assist them in achieving their goals outlined in their NDIS funded plan by proactively identify, develop, coordination and facilitate access to supports and services in their community.

This position requires a highly organised person with excellent oral, written and interpersonal communication skills. We are looking for a person who can work independently and cooperatively as a member of a team and able to manage their own time effectively.

We are looking for a person who has positive and contemporary values towards people with disabilities and must have the ability to interact communicate and engage effectively with diverse cultures.

We are looking for someone who understand the complexity of the NDIS and how the system works. This person has to be great with networking in the sector. We work alongside various government departments including Health, Education, Guardianship Mental Health, Justice, Housing and Public Trust.

Key Responsibilities:

  • Work with clients, families and carers to identify and access opportunities for skills development, education and employment.
  • Support and encourage clients to achieve their goals and provide support coordination services in line with their NDIS plan.
  • Develop networks and collaborate with stakeholders to proactively identify plan, implement and evaluate supports.
  • Prepare and maintain up to date and accurate notes, reports using clear and concise language on clint progress.
  • Attend and contribute to team meetings, discussions and planning.

You will require the necessary clearance cards including NIDS worker screening and Blue Card and also completed worker orientation module. We pay under the SCHADS award for our experienced Support Coordinators starting at a level 4.1.
