Assist NDIS participants in the support coordination and implementation of supports across their funded plan to strengthen and enhance the client’s abilities and goals. This is a full time, 12-month maximum term contract with flexibility to work from home. Travel will be required across the Hunter region to successfully deliver the clients plan and meet their obligations.
Successful candidates will be required to clear all state-based probity checks; including but not limited to a National Criminal History Record Check and NDIS Worker Screening Check.
