This job will have a responsibility to coordinate the roster of the Complete Care program and contribute to further extension of the current and develop intake strategies that will assist the organisation to grow and expand.

Responsibilities:

• Maintain Complete Care roster on the Telstra Health program
• Provide direct training and opportunities for training, supervision as required
• Monitor direct care staff compliance within MCWA and departmental guidelines
• Apply ethical decision making processes according to relevant professional and program standards to effectively support consumers. Work within boundaries of professional areas of competence and consult the Complete Care and Intake leader if required.
  Intake
• Respond to MAC referrals determining eligibility and priority for available programs at MCWA
• Manage service request waitlists to ensure maximum occupancy in relevant program
• Provide information and advice to consumers about service options.
• Undertake pre- home risk assessment process
• Where a referral is not suitable provide information and advice to consumers about alternative service options
• Identify opportunities and contribute to improve Access and Allocation processes and practice to enhance the efficiency and effectiveness of services.
• Positively contribute to the DOH compliance cycle ensuring practice is in accordance with organisational guidelines and meets the required Home Care Standards.
• Employ strategies to ensure effective risk management across the service and report concerns to the Complete Care and Intake Leader
• Maintain and report on statistical data that enhances service options and program development
