CARE ADVISOR – HOME CARE PACKAGES

We are looking for an experienced and motivated Care Advisor to join our Home Care Packages Team. This role is instrumental in providing support and advice to the Home Care Packages clients to assist them to continue living at home. Your responsibilities will include day to day management of a client case load, conducting assessments, care planning and provision of consumer directed services to complex care clients within budget guidelines. You will build professional relationships internally and externally and look for creative care solutions to achieve the best outcomes for clients. This role involves travelling to meet clients in the community as well as working in a busy office environment.

What you bring:

  • Well-developed communication and negotiation skills.
  • Advanced planning, organisational and time management skills.
  • Proven ability to establish effective and cooperative internal and external working relationships.
  • Experience in assessment, case management and service delivery to aged clients living in the community.
  • Strong experience in managing client budgets.
  • High level problem solving and analytical skills.
  • Exceptional customer service skills.
  • Ability to work independently and as part of a team.
  • Tertiary qualification in Social Work, Allied Health, Nursing, Case Management or equivalent.
  • Excellent IT skills, including Microsoft Office suite.
  • Current drivers’ licence.

Desirable:

  • Experience in working with Home Care Packages clients.
  • Knowledge of Carelink+ would be an advantage.
  • Demonstrated experience in sales and packages maximisation to meet client needs.
