Support Coordination
We deliver high quality support coordination to people with a disability and their families. Support Coordination aims to build individual capacity to:
- Manage and implement the participant’s plan
- Achieve the outcomes identified in the participant’s plan
- Connect the participant with formal, mainstream and funded supports.
Being a valued member of the support coordination team, you will go above and beyond to strengthen our participants’ abilities to coordinate and implement supports and participate fully in the community. This includes initial assistance with linking participants with the right providers to meet their needs, assistance to source appropriate supports, coordinating both funded and mainstream supports, resolving points of crisis, and developing participant capacity within their own network and community.
About the role
As a support coordinator you would be joining a large supportive team who work across four sites – Thornbury, Boronia, Werribee and Bendigo. Every day is different, but a typical day could include:
- Making phone calls to participants, families and external service providers
- Writing case notes and documenting progress towards goals
- Writing and submitting comprehensive reports
- Liaising with colleagues and discussing service options for participants
- Liaising with key stakeholders including Local Area Coordinators and NDIS Planners
- Attending and facilitating meetings
- Budgeting and breaking down participants funding
- Completing referrals
- High attention to details and ability to multi-task
About you
- Tertiary qualification and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice)
- NDIS awareness, particularly in relation to support coordination and the importance of choice and control for participants
- Demonstrated ability to develop positive relationships with a range of participants, their families, and their service providers
- Well-developed communication (both oral and written) skills with the ability to prepare reports and correspondence in clear and concise language.
- Strong interpersonal skills with an understanding of diversity when working with participants and colleagues
- Strong organisational skills with the ability to manage a high caseload and meet KPI targets relating to the billing of service delivery
- Intermediate computer literacy with exposure to client management systems (preferred)
- Ability to be flexible in a changing environment. Embrace change and be innovative
- Show initiative and be proactive in the approach to your work
- Enthusiasm and passion for providing quality support to participants and families
