Meliora Life is a boutique company with a bespoke approach to service delivery and we specialise in providing NDIS support to those living with a psychosocial disability.

We have an opportunity for a Support Coordinator/Psychosocial Recovery Coach to join our team in a part time capacity (3 - 4 days/ week) to support participants in Southeast QLD.

Role Overview:

  • Location: Office is in Brookwater QLD, Coordinator will have the flexibility to work at home partly. They will be required to work in the office a couple of days a week.
  • Position: Permanent part-time 30 hours per week.

As a Support Coordinator/ Psychosocial Recovery Coach, you will play a crucial role in assisting individuals with disabilities and their families in navigating the National Disability Insurance Scheme (NDIS). You will work closely with participants and other stakeholders to connect them with appropriate services and resources. Your expertise and guidance will empower them to achieve their goals and maximise their independence.

Key Responsibilities Includes but not limited to:

  • Working collaboratively with participants, their families, and various stakeholders including the NDIA, Office of the Public Guardian, Public Trustee, NDIS Service Providers, and Community and Mainstream services (Housing, Health, Justice, Education).
  • Assisting participants in developing their understanding of funding, implementing their plan, and empowering them to exercise choice and control in selecting their supports.
  • Supporting participants in setting and working towards their short and long-term goals, while continuously monitoring and reporting their progress.
  • Assisting participants in resolving conflicts or challenges that may arise during their journey.
  • Monitor the quality and effectiveness of service delivery.
  • Management of multiple and complex supports from a range of providers.
  • Preparing and providing written reports as needed to support participant plan reviews.
  • Gathering reports from service providers to facilitate participant plan reviews.
  • Ensure service agreements and budget arrangements are in place with appropriate parties.
  • Responding promptly and professionally to inquiries from participants and other stakeholders.
  • Providing ongoing individualised Support Coordination services based on the unique needs of each participant.
  • Establishing and nurturing positive relationships with local community groups, NDIS services, and Mainstream services through networking and collaboration.
  • Attending stakeholder and networking meetings to stay informed and connected.
  • Supporting participants in engaging with services and facilitating service agreements Reach agreements with providers in consultation with the Participant

Requirements:

  • Demonstrate understanding of and commitment to WHS legislation and standards.
  • Tertiary qualifications in Human Services, Community Services, Disability, Mental Health, or other related discipline.
  • Be fluent in the NDIS Quality and Safety Commission standards and legislation as it relates to quality disability service provision and the relevant service classes.
  • Significant knowledge and experience in the disability field / community services and demonstrate strong knowledge of the NDIS.
  • Experience supporting a broad range of participants with varying needs.
  • Ability to work independently with minimal supervision.
  • Possess problem solving, planning and organisational skills and be detail oriented.
  • Agile in resolving issues and conflicts.
  • Knowledge of standards and regulations for disability services.
  • Experience in crisis management and risk mitigation.
  • Current Class C Driver’s Licence
  • Current COVID-19 Vaccination Status
  • National Police Check
  • NDIS Disability Worker Screening Check
  • QLD Working with Children Check (Blue Card)
  • Access to a roadworthy vehicle with insurance and current registration
  • Must have working rights to work in Australia.
