Menora Foundation is looking for a compassionate and dedicated Community Support Worker / Mental Health Worker to join our small community based team primarily working within the Perth Jewish Community.
Primarily focusing on working collaboratively with clients to assess their unique needs, strengths, and support networks, this role concentrates on understanding clients’ current circumstances and goals and empowering them to develop and implement personalised plans to address challenges and enhance their overall well-being.
The role includes individual and family informal therapy, case co-ordination, group work and referral.
Key Responsibilities
Working under the direction of the Senior Social Worker
- Work collaboratively with clients to explore their unique needs, strengths, and support networks, supporting them to identify their goals and develop tailored responses that promote their well-being and self-determination.
- Provide emotional support and guidance to individuals facing mental health challenges.
- Connect individuals with relevant support services and community resources.
- Work collaboratively with healthcare professionals, social workers, and local organisations.
- Provide client-focused support and services using a Strength-Based and Person-Centred Active Support Model to empower clients, promote engagement, and foster involvement in developing and implementing recovery or action plans for future goals.
- Maintain accurate case notes and ensure confidentiality.
- Liaise and work jointly with colleagues and staff from within Menora and external to Menora and liaise with other agencies on behalf of clients and Menora.
Qualifications & Skills
- Certificate III or IV in Mental Health OR Certificate III or IV in Community Services (desirable).
- Experience in community outreach, mental health, or social services preferred.
- Strong communication and active listening skills.
- Ability to build trust and rapport with diverse individuals.
- Knowledge of local mental health resources and support networks.
