Mental Health Support Worker - Social Support Programs - Mornington

Mentis Assist's Social Support Programs focus on increasing the independence of our clients by connecting them to community and other support services. In accordance with the clients' Care Plans duties may include:

  • providing transport to activities and services
  • providing support to complete household tasks
  • meal planning and preparation
  • assisting with access to exercise and fitness activities
  • shopping
  • decluttering
  • appointment booking

Key requirements:

  • experience in a similar person centred, recovery focused role
  • Certificate III in either Individual Support, Aged Care/Disability/Home and Community Care or Certificate IV in Mental Health
  • Current First Aid Certificate (or willing to undertake)
  • Minimum 18 months experience working with clients with mental health presentations
  • Evidence of full COVID 19 vaccination
