We have an opportunity for a Case Worker to join our Reconnect Team within our Youth Accommodation Team on a permanent full-time basis.
This role is a community-based early intervention and prevention program for young people aged 12 to 18 years who are homeless or at risk of homelessness, and their families. The role will provide culturally appropriate counselling, mediation and case management through proactive community outreach with a focus on Aboriginal and Torres Strait Islander young people.
You’ll have significant experience in working in the Social Service Sector, and an understanding of evidence-based service models for working with youth and their families. Along with this, you’ll also have:
