Job Description

The Carefinder Program is a part of consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that targets people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or other relevant supports in the community.

Aged Care Navigators provide specialist and intensive assistance to help people understand and access aged care and connect with other relevant supports in the community.

The Program intends to achieve the following outcomes for people:

• Improved coordination of support to access aged care.
• Improved understanding of aged care services and how to access them.
• Improved openness to engage with the aged care system.
• Increased rates of access to aged care services and connections with other relevant supports.
• Increased rates of staying connected to the services they need post service commencement.
• Improved integration between the health, aged care, and other systems at the local level.

This is a fixed-term (30/06/2025) part time (30.4h/week) employment opportunity with conditions and salary at Level 5 of the SCHADS Award.

Desired Skills and Experience

Key Responsibilities

• Provide support to individuals in understanding and accessing aged care services and community resources.
• Conduct assertive outreach to engage and build relationships with participants.
• Perform comprehensive assessments to identify needs and ensure appropriate referrals to services.
• Collaborate with service providers and maintain continuous support for participants until services are in place.
• Work effectively within a team and alongside external stakeholders to ensure coordinated care and service delivery.

Essential Criteria

• Demonstrated commitment to child and vulnerable adult safeguarding principles.
• Relevant qualifications and experience, or a combination of expertise and competence.
• Strong understanding of trauma-informed care and experience working with disadvantaged individuals.
