The Families to Home Team provides outreach support to families with children under 18 accommodated in emergency accommodation throughout Brisbane. Through outreach and coordination with other teams within Micah Projects, the team provides access to crisis accommodation whilst supporting the family to develop a Housing Pathway Plan that considers the needs of all members of the family

As a Family Pathways Outreach Worker you will work as part of the Families to Home Crisis Team to work with families experiencing homelessness. To assist families in crisis accommodation to access affordable and permanent housing through proactive outreach and crisis case management.

This is a permanent full-time employment opportunity with conditions and salary at Level 4 of the SCHADS Award.

Key Responsibilities

  1. Immediate Assistance: Provide immediate help to families facing homelessness, including crisis motel accommodations.
  2. Homelessness Prevention: Offer financial aid and referrals to sustain tenancies and prevent homelessness.
  3. Referral Coordination: Coordinate referrals to specialist homelessness services for accommodation and support.
  4. Proactive Outreach: Engage with families in motels, prioritizing housing pathways and urgent needs.
  5. Housing Applications: Ensure completion of housing application forms (Rent Connect or Social Housing) for each family.
  6. Relationship Building: Develop trusting relationships with families to identify support needs affecting housing outcomes.
  7. Interagency Collaboration: Partner with the Department of Housing and Child Safety, and coordinate with Domestic Violence services as needed.
  8. Team Collaboration: Work within a team to manage engagement and oversight of families, maintain real-time data entry, and use appropriate tools for referrals and reporting.

Essential Criteria

  1. Experience and Knowledge: Demonstrated experience working with disadvantaged and homeless individuals/families, including Aboriginal, Torres Strait Islander, and culturally diverse backgrounds.
  2. Qualifications: Relevant certificate, diploma, or tertiary qualification with extensive experience, or a combination of experience and competence.
  3. Communication and IT Skills: Effective communication strategies, interpersonal skills, problem-solving, advocacy, computer literacy, and proficiency in IT systems (including Microsoft Office).
  4. Adaptability and Teamwork: Positive attitude, flexibility, and ability to work both autonomously and within a team, adhering to program guidelines and Micah Projects' values.
