We are seeking a compassionate, self-motivated Community Mental Health Practitioner eager to make a difference in people’s lives for a permanent, full-time opportunity to work alongside our Psychosocial Support services in the Adelaide CBD area. You will provide person-centred, recovery-oriented support to clients, helping them achieve their recovery goals, build capacity to live safely in the community, strengthen social connections, achieve wellbeing goals and live independently.
Mind’s Community Services in South Australia provide person-centred, psychosocial support, including goal setting, transition planning, and assistance with application and transition to NDIS services. The services comprise a two-tiered approach to support:
Provide flexible, intense support and case management for a limited period (6 weeks) to reduce immediate distress, enhance personal safety, and connect clients to long-term community services.
Offer moderate support over a four-month period (extendable to 12 months) to help clients achieve recovery goals, build daily living skills, maintain housing, and foster social connections. Assist clients with NDIS eligibility, application, and transition.
