About the role

The NAPSS ICSS Psychosocial services provide person-centred support, including goal setting, transition planning, as well as application and transition to NDIS services. The services employ a two-tiered approach to support:

  • Tier 1: Intensive Crisis Support offers flexible, intense support and case management for a limited period (6 weeks) to reduce immediate distress, enhance personal safety, and establish links with longer-term community services.
  • Tier 2: Non-acute Psychosocial Support offers moderate intensity support over a four-month period (extendable to 12 months). This support focuses on achieving My Better Life goals, enhancing recovery, building daily living skills, maintaining sustainable housing, improving relationships, developing social connections, and living independently. Additionally, it assists clients in assessing NDIS eligibility, applying for access, and transitioning to NDIS services.

We are seeking a compassionate, self-motivated Community Mental Health Practitioner eager to make a difference in people’s lives for a permanent, full-time opportunity to work alongside a caring team in the Port Augusta & Whyalla area. You will provide person-centred, recovery-oriented support to clients to achieve their recovery goals, build capacity to live safely in the community, strengthen social connections, achieve wellbeing goals and live independently.

Key responsibilities

  • Deliver high quality services and facilitate positive educational and social experiences.
  • Provide support to clients using targeted and integrated case managed responses.
  • Support clients to return to their community and enjoy a meaningful life through an individualised approach to recovery.
  • Provide information and mentoring to clients to establish relationships with families and carers, create community networks and access to relevant services.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Awareness and understanding of the NDIS is desirable.
  • Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
