About the role

The Rockingham Sub-Acute short stay residential service or Kwelena Step-Up Step-Down service (KSUSD) is a 10-bed service offering short term stay up to 30 days. We support clients with psychosocial disability to transition from an acute hospital setting or, prevent hospital admissions by providing extra support to those entering directly from the community.

 We are seeking a self-motivated Community Mental Health Practitioner for a casual relief opportunity in the Shoalwater, Rockingham area. You will work collaboratively with clients to achieve their recovery goals through one-to-one support, shared supports, workshops and group work facilitation to enhance recovery, personal growth and activities of daily living.

Key responsibilities

  • Purposefully engage with clients using brief intervention, family inclusive practice, trauma informed practice, behavioural support.
  • Support family and carer roles through understanding their concerns and the provision of information, education and referrals.
  • Make linkages and build relationships and referral pathways to maintain or create a range of local supports.
  • Document all activities using Mind’s ICT system and processes.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Demonstrated understanding of available community services, networks and supports.
  • Excellent interpersonal and communication skills with the ability to consult, negotiate and influence
  • Ability to work both autonomously and collaboratively showing initiative and flexibility.
