Mind is opening a Supported Independent Living (SIL) Haven in Geelong!

About the role

Mind operates Supported Independent Living (SIL) for people with severe and enduring psychosocial disability. Geelong will accommodate 16 residents, the Community Mental Health Peer Practitioner will support the team 30.4 hours per week across a 24/7 rotating roster, ideally with great availability over morning, afternoon, evening and sleep over shifts.

Services operate in the context of the local community, with a focus on supporting people to achieve their better life goals through Mind’s My Better Life plans and activities of daily living skills.

What you will bring

  • Proven experience and expertise in working directly with people with mental ill health issues, complex needs, and with their families and carers.
  • Experience in completing case notes and relevant documentation in line with service requirements
  • Ability to plan and prioritise to meet customer service delivery requirements
  • Understanding of available community services, networks and supports.
  • Support and work effectively with clinical services
  • Experience in working with people with an intellectual disability or ASD is desirable
  • Passion for working in a solutions focussed team to achieve service delivery outcomes

What are we looking for?

  • Awareness and understanding of NDIS and local community services networks and supports
  • A team player with a focus on a positive team culture and assisting customer’s reach their goals using evidence informed practice
  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field as designated by Mind
