About the Role

We are seeking self-motivated, compassionate Support Coordinators for fixed-term, part-time roles (30.4 and 22.8 hours/week) to work alongside a dynamic team in the Melbourne area. Working from home, you will provide Level 2 Support Coordination to participants with complex needs to initiate and coordinate their NDIS plan, negotiate appropriate support, access a range of mainstream services and community support and develop their skills and confidence to manage their plan independently. In this role you will apply the principles of choice and control by ensuring awareness and options of services are offered to the participants.

About the role

The Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have NDIS plans and/or TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model. The FFS program provides centre based and tele-health services, outreach services, support coordination, and education and vocational support. The service offers clients choice and control over services so they are able to build life skills and capacity, develop social connections and create community linkages to identify and achieve their recovery goals.

Key responsibilities

  • Deliver person-centred, timely support coordination services consistent with defined goals and aspirations identified in NDIS plans.
  • Provide an exceptional customer experience ensuring service delivery needs and wants are met by external service providers within funding budgets.
  • Actively manage multiple/complex supports from a range of providers and make ongoing adjustments when required.
  • Support participants to develop skills and build capacity to understand and manage their NDIS plans.
  • Collaborate with a range of services, families and the community to ensure a co-ordinated, integrated response to client’s recovery goals. 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capacity to plan and prioritise meeting customer service delivery requirements.
  • Demonstrated understanding of NDIS including the NDIS price guide and legislation, with ability to build strong relationships with service providers, clinical teams and key stakeholders.
