We are seeking self-motivated, compassionate Support Coordinators for fixed-term, part-time roles (30.4 and 22.8 hours/week) to work alongside a dynamic team in the Melbourne area. Working from home, you will provide Level 2 Support Coordination to participants with complex needs to initiate and coordinate their NDIS plan, negotiate appropriate support, access a range of mainstream services and community support and develop their skills and confidence to manage their plan independently. In this role you will apply the principles of choice and control by ensuring awareness and options of services are offered to the participants.
The Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have NDIS plans and/or TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model. The FFS program provides centre based and tele-health services, outreach services, support coordination, and education and vocational support.
