We are seeking a highly motivated, experienced Team Leader for a permanent, part-time opportunity to work at the Step-Up Step-Down residential service in Bundaberg. You will provide leadership and support a team to deliver person-centred, recovery-oriented supports to clients to develop daily living and social skills, build community networks, achieve wellbeing goals and live independently in the community. The Team Leader will act as the practice lead to staff providing supervision, mentoring and coaching.
The Sub-Acute short stay Adult Step-Up Step-Down (ASUSD) residential service, delivered in partnership with QLD hospital networks and the local health service, supports adults with a psychosocial disability up to 28 days transitioning from a hospital setting back into the community or to prevent admissions whilst residing in the community. ASUSD works within a recovery focused, clinical treatment framework providing personalised one-on-one support, shared supports and group activities based on client’s strengths and goals as identified in their recovery plan
