About the role

The Assertive Outreach program is a flexible and highly coordinated approach to providing intensive case management support and healthcare to individuals in the community who are experiencing primary homelessness. The program focuses on engaging with people in a respectful, non-judgmental manner within their environment, meeting them where they are at, and supporting reintegration back into the community. The program aims to facilitate rapid housing pathways while offering advocacy and referrals to a broad range of multidisciplinary services.

You will have

At least 3 years’ experience in case managing clients with complex needs, at risk and/or experiencing homelessness, including sound knowledge of integrated case management theory and best practice principles.

Recognised Tertiary qualifications in Social Work, or a relevant field.

Key selection criteria

As a Senior Case Manager, you will provide intensive case management through assertive outreach, working closely with clients in their environment. Please focus on addressing the key selection criteria, highlighting the skills and experience required for the role. Below are four key points to consider:

Experience in Homelessness and Complex Case Management

  • Demonstrated experience in case management, specifically working with homeless and/or vulnerable peoples, including those with complex needs such as mental health, substance abuse, and trauma history.
  • Ability to engage with and support individuals in crisis, ensuring their immediate and long-term needs are met through coordinated care.

Assertive Outreach and Client Engagement Skills

  • Proven ability to conduct assertive outreach, managing safety and risk, meeting clients where they are, building trust, and establishing rapport in challenging environments.
  • Capacity to engage clients who may be disengaged from traditional service models and provide consistent follow-up and support.

Strong Communication and Collaboration Skills

  • Excellent communication skills, with the ability to engage clients, team members, and external service providers effectively.
  • Ability to collaborate with a multidisciplinary team, including housing, mental health, and health services, to deliver holistic and person-centered care.
  • Maintain accurate written records, statistics and reports.

Cultural Sensitivity and Trauma-Informed Approach

  • Knowledge of and sensitivity to the diverse backgrounds of individuals experiencing homelessness, including cultural, socioeconomic, and gender considerations.
  • Commitment to using trauma-informed practices to ensure a safe and supportive environment for clients.
