Monash Health Community actively engage with people to improve and maintain health, independence and well-being. We do this by coordinating and/or providing integrated, multidisciplinary care across multiple settings within the local community. Our aim is to help clients live their best lives by enabling them to access the support they need, when they need it., supporting clients through all stages of non-acute care from prevention through to rehabilitation.
The Primary Care Support Program cares for clients who have complex needs and may require one or more discipline of care. The team comprises of Allied Health, Psychology, Nursing, and Administration support; with the client’s GP being central to communication and care planning.
Working closely with the Director of Community Assessment and Response, this newly created leadership position manages the daily operations of the team to ensure the delivery of high standard and evidenced-based care in a community setting.
The Allied Health Manager, Primary Care and Support is integral in developing and leading an innovative multidisciplinary team and contribute to the changing landscape of community care. You’ll play a key role in developing the program, as well as leading and supporting a team of nursing and allied health professional who deliver care in the home and centre-based settings so that clients receive the service that best meets their needs.
In this full-time ongoing role, working Monday to Friday, you will based in Cranbourne at the Cranbourne Integrated Care Centre. As you’ll be travelling to other sites as required, a valid drivers licence is essential.
You are an experienced Allied Health Professional with a proven track record in leading a team to achieve its goals and are ready to take this next step in your career.
