Our aim is to help clients live their best lives by enabling them to access the support they need, when they need it., supporting clients through all stages of non-acute care from prevention through to rehabilitation.

The Primary Care Support Program cares for clients who have complex needs and may require one or more discipline of care. The team comprises of Allied Health, Psychology, Nursing, and Administration support; with the client’s GP being central to communication and care planning.

A Day in the Life:

Working closely with the Director of Community Assessment and Response, this newly created leadership position manages the daily operations of the team to ensure the delivery of high standard and evidenced-based care in a community setting.

The Allied Health Manager, Primary Care and Support is integral in developing and leading an innovative multidisciplinary team and contribute to the changing landscape of community care. You’ll play a key role in developing the program, as well as leading and supporting a team of nursing and allied health professional who deliver care in the home and centre-based settings so that clients receive the service that best meets their needs.

In this full-time ongoing role, working Monday to Friday, you will based in Cranbourne at the Cranbourne Integrated Care Centre. As you’ll be travelling to other sites as required, a valid drivers licence is essential.

You are an experienced Allied Health Professional with a proven track record in leading a team to achieve its goals and are ready to take this next step in your career. You lead by example, thrive working in a dynamic, fast paced environment and are truly passionate about delivering a high standard of evidence-based care to all clients, all the while, advocating for Monash Health’s six guiding principles.

What you bring:

  • Ability to engage in reflective practice to support extensively specialised clinical care delivery
  • Demonstrated experience in identifying need for change and leading others through this process, using extensive specialised knowledge
  • Work autonomously and collaboratively with the multidisciplinary team to deliver safe consumer focused care
  • Demonstrated advanced leadership skills and experience skills in people management, conflict resolution and building and motivating teams
  • Demonstrated advanced knowledge, leadership and experience in quality improvement, research processes and commitment to achieving best practice professional clinical standards
  • Extensive specialist knowledge and experience to develop and lead the professional development of others at undergraduate, professional and postgraduate levels
  • Demonstrated knowledge and sensitivity to ethical and cultural issues and vulnerable population groups, integrated into practice
  • Organisation, time management, planning and priority setting
  • Excellent written and verbal communication skills.

What you need:

  • Tertiary qualification in an Allied Health health-related discipline
  • Registration with an appropriate Board
  • Significant experience post registration/qualifying experience
  • Desirable – postgraduate qualification in a relevant field.
