Our Journey

The Monash at Home – Aged and Rehabilitation Care service, previously GEM at Home, launched in December 2020 and is a fundamental part of the broader Aged and Rehabilitation Division, providing safe and high-quality home-based subacute level care for individuals with complex medical, functional, and often cognitive issues, who require comprehensive, medically led multidisciplinary assessment and optimisation.

The service is seen as an alternative to hospital-based subacute care, designed to facilitate an earlier discharge from acute care and/or to avoid a subacute hospital stay entirely. Patients within the service receive at least daily visits from a member of the multidisciplinary team and access to 24-hour telephone support. The model of care aims to provide short term, high quality, safe and effective patient centred care to individuals within the primary Monash Health catchment. The Program is entering an exciting phase of growth which will include, but not limited to, increasing the service clinical capacity and extending the geographical catchment.

The Monash at Home Aged and Rehabilitation Care Service operates out of Kingston Centre and will be opening up a second site at Cranbourne at the beginning of October. It is expected that this position will be required to work across both the Kingston and Cranbourne site

A Day in the Life

The Grade 3 Podiatrist is a senior clinical expert and skilled clinician with consolidated leadership, extensive podiatry assessment and planning skills and able to use evidence-based information to inform practice and function as a pivotal member of the multidisciplinary team in the Monash at Home – Aged and Rehabilitation Care service.

You are expected to provide best practice and excellent service delivery to our patients. You will work effectively and collaboratively while developing strong partnerships both within Monash Health, and outside the organisation. The core function of the role includes the following:

  • Client assessment, planning, treatment and coordination of services to patients
  • Consistent re-evaluation of protocols and practices and patient care to ensure best practice.
  • Participate in development of a collaborative Multidisciplinary approach to achieve patient goals in care
  • Flexibility to work across any clinical area at any or all campuses of Monash Health as required.

What You Bring

You are an accomplished Podiatrist with highly developed clinical expertise in high risk foot care and wound management. You have exceptional clinical knowledge and assessment skills, and ability to work with a range of stakeholders.  You have a genuine passion for delivering clinical care in the community setting and have demonstrated clinical leadership in this field. You have proven ability to deliver successful, timely outcomes across service delivery, and you are an effective communicator who builds rapport rapidly and whilst you can work autonomously, you are also a strong team player and work efficiently in a team environment and are passionate to achieve outstanding outcomes for our consumers.

On a personal level, you can also demonstrate:

  • Innovation and expertise in delivering hospital level care in the home
  • Commitment to supporting your manager and the team in a leadership and mentoring role
  • Proven time management, superior communication and influencing skills
  • Critical thinking, analytical and problem-solving skills

What you need:

  • Bachelor of Podiatry, Bachelor of Applied Science (Podiatry) or recognised equivalent qualification
  • Current Drivers licence without restrictions
  • “Completion or working towards a post Graduate qualification in relevant field” will be highly regarded
  • Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA) - Podiatry Board of Australia, with no restrictions.
  • Achieve and maintain competency in basic life support/first aid skills in accordance with the AHPRA Registration guidelines
