Monash Health
  • Work with an experienced team of Social Workers & Lived Experience Workers
  • Provide post-discharge assertive outreach, follow-up and advocacy
  • Full time or part time, ongoing positions available

Working in Healthcare has never been more important or meaningful.

Our Journey

The Engage Team commenced at Casey Hospital mid-2021 and it has since expanded to Dandenong ED at the beginning of 2022. The team initially consisted of Senior Social Workers and now also includes Consumer Peer Workers. We are seeking a Grade 3 Social Worker to join the Mental Health Program in the Dandenong and Casey Emergency Departments (ED). In this role you will provide an innovative service for people presenting to the ED requiring complex psychosocial assessment, support and assistance to ensure an appropriate discharge back to the community.

A Day in the Life

Working within the ED is exciting and rewarding – no two days are the same. You will provide advanced clinical expertise to mental health consumers, their families and carers to maximise individual recovery. Key features of the role include:

  • High quality assessment, targeted interventions and advocacy that are tailored to individual needs to support a safe discharge.
  • Providing a month of post-discharge assertive outreach, follow-up and advocacy to patients.
  • This role will suit you if you are ready for a new challenge, utilising the experience you already have and developing your career in a new and exciting direction.

This is an opportunity to work in a unique team made up of social workers and peer workers dedicated to supporting people with complex psychosocial needs within the fast paced emergency setting. You demonstrate exceptional communication, collaboration and problem solving skills to achieve excellent outcomes.
