Our Cancer Care Centre is seeking to engage a Clinical Nurse Specialist in the role of Gastrointestinal (GIT) Cancer Care Coordinator on a temporary assignment. This position is responsible for improving the continuity of care, identifying issues, conducting psychosocial assessment, intervening early with attempts to avoid hospital admission or reduce length of stay amongst patient cohort for curative, adjuvant, and metastatic GIT oncology patients.
The successful incumbent will work closely with other Care Coordinators and Oncologist affiliated with Nepean Cancer Services. The role acts as the primary case manager for complex episodes ensuring the proper monitoring and evaluation of the patient’s care pathway in relation to service provision and complaints.
