About Us

Nepean Blue Mountains Local Health District (NBMLHD) is a wonderful place to expand your career and grow your skills and knowledge. As a recognised leader in the healthcare industry, we provide a range of public health services to the Nepean, Blue Mountains, and Lithgow Region.

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Our Community Supportive Care service aims to promote and support a client’s function and independence at home whilst providing support to their family and carers. An opportunity has become available for a qualified Occupational Therapist to join our interdisciplinary team. This position works with other community health teams, general practitioners, hospital teams, and specialist palliative care services to co-ordinate and deliver best supportive care to clients with late stage degenerative and chronic conditions within the last 2 years of life.
As an Occupational Therapist, you will develop measurable and realistic goals, care plans, and priorities to ensure timely clinical intervention. Together with the team, clients, and carers; the Occupational Therapist will implement and review the care plan and provide education and information regarding ongoing intervention, equipment, and community resources whilst ensuring close liaison and timely referrals to other team members and service providers.

The successful incumbent will demonstrate advanced reasoning skills and ability to work autonomously with minimum direct clinical supervision when providing care to the clients of the service. Selection Criteria

  1. Degree in Occupational Therapy and current registration with Occupational Therapy Board of Australia (AHPRA).
  2. Demonstrated high level of clinical expertise and knowledge in the assessment and management of clients with chronic and complex conditions including experience in palliative care across community or hospital settings.
  3. Demonstrated leadership ability to provide clinical support, education, and consultation to health professionals in area of specialty to improve client outcomes and service design.
  4. Demonstrated problem solving and decision-making abilities according to best practice including the ability to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
  5. Demonstrated ability to communicate effectively with clients, carers and stakeholders and provide advice to managers on service development.
  6. Demonstrated commitment to evidence-based practice and quality improvement.
  7. Ability to work independently with minimal supervision and maintaining involvement within the wider interdisciplinary team.
  8. Current driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
