As the Clinical Lead, you will lead a team of clinicians and be responsible for providing clinical oversight to the transdisciplinary mental health teams of the BS MMHps. This transdisciplinary team includes Intake and Assessment Coordinators (including Lived Experience), Mental Health Clinical Support Coordinators (including mental health registered nurses) and a Peer Practice Lead. You will report to the Service Manager for the Darling Downs West Moreton and Brisbane South Medicare Mental Health Intake Assessment and Referral Services.
As Clinical Lead, you will guide the delivery of high-quality mental health intake, assessment, triage, and referral services for individuals in the community who experience mental health issues and are seeking support. A priority of the role is to connect people seamlessly to the most appropriate local service to meet their identified needs, consistent with the 'stepped care' approach. You will be responsible for ensuring continuous care co-ordination between Metro South Health Services as well as other health service providers for consumers experiencing high levels of distress. The Clinical Lead will have oversight of the IAR-DST, clinical assessments, collaborative care plan formulation and execution as well as escalation and crisis support protocols.
The Medicare Mental Health phone service (MMHps - formally Head to Health Phone Service) is an exciting new service connecting the Brisbane South communities to the most appropriate services that meet their identified needs. When calling the 1800 number, consumers will be greeted by mental health professionals offering mental health service navigation into and between all community mental health services. This consists of Medicare Mental Health Centres, Headspace, and other community-based mental health services, including those provided by Queensland Health, Non-Government Organisations, and private providers.
The MMHps is a part of a national Medicare Mental Health brand, that enables a consistent triage and streamlined transfer or referral to the most appropriate services. A key objective of this service is local service integration to address fragmentation and offer a seamless care pathway for consumers to receive the right level of care at the right time to meet their mental health needs.
This is primarily a phone-based service, taking direct incoming referral calls through an established 1800 mental health intake number. The service uses the Initial Assessment and Referral Decision Support Tool (IAR-DST) to support a nationally consistent evidence-informed initial assessment and referral process. The team is made up of staff from diverse professional and personal backgrounds including clinical, non-clinical, and lived experience. Staff work together in a transdisciplinary approach to support people to reach the appropriate service, prioritising consumer choice and agency.
The Brisbane South MMHps program is delivered in partnership with Metro South Health and Brisbane South PHN (based in Logan). The tripartite partnership is designed to complement - not replace or duplicate - mental health support lines and services already provided in the Brisbane South community. These existing services include crisis support lines and state-funded triage services facilitating intake into tertiary services.
The MMHps operates from 8:30 am to 5 pm Monday to Friday. Opening hours may be adjusted over time in response to community needs and service demands.
