  • Opportunity to coordinate a specialist clinics project across Victorian Primary Health networks
  • 0.8 FTE (30.4 hrs/week), Maximum Term role until December 2025
  • Salary starting approx. $86,000 (Pro-rata) + superannuation + eligibility for salary packaging
  • Generous NFP salary packaging options up to $18,550
  • Docklands office location with option to work some days at home

Reporting to the Director, Integrated Care, the Lead, Specialist Clinics Reform Project will coordinate and oversee the development, implementation and evaluation of the Specialist Clinics Project by Victorian Primary Health Networks, in partnership with the Victorian Department of Health and Human Services (DHHS).

NWMPHN is lead agency for the Victorian Specialist Clinics Project on behalf of the Victorian and Tasmanian Primary Health Network Alliance (the Alliance). The Project commenced in 2018 to support the implementation of the Victorian specialist clinic referral criteria into primary care and is now moving in its sixth tranche of project activity.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • Increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • Improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Designing and coordinating statewide HealthPathways or online care pathways development, implementation and evaluation activities
  • Managing the project using rigorous project management methodology and systems and prepare and identify potential risks Prepare key deliverables including requisite reports for DHHS and monthly reporting to Victorian PHN CEOs
  • Working with contracted organisations to ensure services delivered are evidence based, client centered and operated in accordance with project guidelines
  • Implementing statewide communication strategies to maintain a high-level of awareness amongst key stakeholders and facilitate information flow.
  • Actively engaging key clinical stakeholders at the statewide level to advise and participate in the implementation of HealthPathways within primary care and ensure broad consultation and appropriate levels of stakeholder engagement

We are looking for someone who has:

  • Experience managing successful multi-stakeholder projects or programs, including managing project teams
  • Relevant tertiary qualifications in a health, project management or related field relevant to the position
  • Demonstrated experience in contract management and developing systems that support the monitoring and delivery of contracts
  • Demonstrated and significant experience in written communication, including the preparation of complex reports to a professional standard
  • Exceptional influencing and relationship management skills including the ability to develop and maintain strong collaborative partnerships
