The Manager, Population Health and Planning is responsible for the strategic and technical management of population health planning and data analytics projects. The role is responsible for leading health needs assessment projects, the establishment of a catchment wide health indicator database, strengthening population health planning methodologies, and driving population health insights for evidence-based decisions and health system planning. The Manager, Population Health and Planning will also lead NWMPHNs responsibilities for the Australian General Practice Workforce planning and prioritisation program.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.
What are PHNs?
PHNs have been established with two key objectives:
- increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
- improving coordination of care to ensure people receive the right care in the right place, at the right time
A typical week may include:
- Provide strategic and technical management of population and planning projects including NWMPHN health needs assessments.
- Lead the analysis and interpretation of population health data to identify local and regional primary health priorities, populations at greatest need and service gaps within the context of current market drivers utilising high level research and epidemiological knowledge and skills.
- Provide leadership across NWMPHN that supports best practice population health planning.
- Provide high level advice on all matters related to population health analysis and reporting of health data, including the impact of government policy change.
- Participate in regular reviews of performance and demonstrate a willingness to close learning and capability gaps.
We are looking for someone who has:
- Tertiary qualifications in public health, population health, health science or a related health discipline. Postgraduate qualifications in a relevant field will be highly regarded
- Demonstrated experience leading population health and health planning projects which require the management, translation and reporting of data and evidence for decision-making.
- Knowledge of current primary care policy and practice environment.
- Proven skills and experience in the management of multi-stakeholder projects.
- Highly developed verbal and written communication skills, including proven experience preparing high quality reports and delivering oral presentations that meet the needs of multiple stakeholders and audiences.
