Open Doors Youth Service provides support to LGBTIQAP+ Sistergirl and Brotherboy young people with diverse bodies, genders, and/or sexualities aged 12 to 24 across South East Queensland.

We are currently looking for an experienced case manager in to provide community based early services to LGBTIQA+ young people aged 12 to 18 who are homeless and at risk of homelessness, and their families, to improve their engagement with family, education, training, employment and local community.

Permanent Part-Time - 3 days per week - 12 Month Contract - SCHADS Level 4 + Salary Packaging.

About the role

As the Reconnect Case Manager, you will be responsible for providing case management, care coordination, goal planning, psychosocial groups, and community based early intervention services to LGBTIQ+ young people aged 12 to 18 who are homeless or at risk of homelessness.

You will work closely with young people and families to improve engaged with family, education, training, employment and their communities.

Centrally based in Fortitude Valley and providing outreach support to across Brisbane and surrounding suburbs.

Key responsibilities of the Case Manager role will include:

  • Providing individual support to young LGBTIQ+ people who are at risk of homelessness through Case Management, Care coordination, and goal planning
  • Develop and delivering psychosocial groups for young LGBTIQ+ people to increase connection to peers and community
  • Develop referral networks and pathways to be accessed safely by young LGBTIQ+ people and their families
  • Working in culturally appropriate ways at all times with young LGBTIQ+ Sistergirl and Brotherboy people including Aboriginal and/or Torres Strait Islander young people and Culturally and Linguistically Diverse young people and families.

About You

The successful candidate will have:

  • Demonstrated experience in a similar role
  • High level of success building relationships and partnerships
  • High degree of internal and external communication skills
  • High level of skill in case management, care coordination and goal planning
  • Robust understanding of the experiences of young LGBTIQ+ people and their families face within systems and communities
  • Experience in developing and executing successful group programs

QUALIFICATIONS REQUIREMENTS

  • Relevant qualifications in the field of Youth Work (Diploma minimum) and/or Degree in Human or Community Services
  • A current driver’s licence
  • A current Queensland Blue Card or ability to acquire a Blue Card
  • COVID 19 vaccination certificate
  • Two relevant references directly linked to experience working in human services

SELECTION CRITERIA

PLEASE NOTE: A written response to the following Selection Criteria is required with your application. Applications without a response to the selection criteria will not be considered.

  1. Knowledge and understanding of the causes of family breakdown and increase the risk of homelessness for LGBTIQ+ young people, and the experiences of LGBTIQ+ young people who are homeless or at risk of homelessness.
  2. Demonstrated ability to provide individual support work to young people, including case management, care coordination and goal planning.
  3. Demonstrated ability to provide support to parents, families and carers
  4. Demonstrated ability to develop and deliver group programs for young people and their families.
  5. Knowledge of relevant referral networks and demonstrated ability to work collaboratively with key stakeholders including schools, government bodies, and community organisations.
  6. Demonstrated ability to work within a collaborative team.

Open Doors Youth Service has been in operation since 2003 providing support to young people who identify as part of the LGBTIQ+ community and their families. We support young people who identify as Lesbian, Gay, Bisexual, Transgender, Intersex, Queer, Asexual, Pansexual, Sistergirl and Brotherboy aged 12 to 24 across South East Queensland.

This is an exciting time to join Open Doors Youth Service. Over the past few years, we have been building our capabilities and services to better cater to the unique needs of young LGBTIQ+ folks in our community. We are a small, but growing group of passionate people who work together to better the lives of our young people.
