About the role

Pathway Essentials is seeking an experienced and passionate Case Manager and Team Leader to join our team in Daylesford, Victoria. In this full-time role, you will drive meaningful change by empowering participants with complex needs, including disabilities, trauma histories, and behaviours of concern.

Collaborating with a multidisciplinary team, you will ensure high-quality, person-centred care. Additionally, you will lead and inspire support workers, providing the mentorship, skills, and resources needed to deliver impactful and compassionate support.

What you'll be doing

  • Case management and support- Manage a case load of participants, ensure participants needs and goals are met, coordinate participant care.
  • Team management and support- Support a team of support workers, facilitate worker training and development, improve workplace processes.
  • Collaboration and stakeholder engagement- Attend care team meetings, collaborate with compliance and operations teams, coordinate with allied health professionals.
  • Intake and Participant Onboarding- Coordinate participant intake, complete necessary documentation, develop and implement support plans,

What we're looking for

  • Bachelor’s degree in Social Work, Youth Work or a related field, or equivalent relevant experience in case management within the disability, youth or health sector.
  • Proven experience in case management, particularly in managing participants with complex needs, behaviours of concern, and substance use issues.
  • Strong leadership skills: Experience in leading or managing teams, with the ability to motivate, support, and develop staff.
  • Excellent communication skills: Ability to collaborate effectively with participants, families, support workers, and external stakeholders.
  • Strong understanding of NDIS processes, policies, and procedures, including participant plans and reporting requirements.
  • Ability to multitask and manage competing priorities in a fast-paced environment, with a keen eye for detail.
  • Commitment to high-quality, person-centred care: A passion for supporting individuals, and their families and a dedication to continuous improvement.
  • Experience in program development highly desirable (job skill readiness, music production or animal assisted learning).
