We are currently seeking a passionate and driven Program Manager - Social Prescribing to join us at PCYC Crestmead. If you are enthusiastic about supporting families, young people and children to gain access to health, social and community services, then we want you!
Reporting to the Youth Programs Operations Manager, you will be responsible for the successful development and delivery of the Social Prescribing - Link Worker Project, which aims to support families, young people, and children by ensuring they are appropriately supported and referred to external agencies and internal services that enhance their specific situation.
