The Community Care Physiotherapist/ Occupational Therapist forms part of Peninsula Health's Health Independence Program and is based on the Wagner Chronic Care Model, an evidenced based systems framework.
The Community Care PT/ OT is responsible for coordinating intensive short term (3-6 months) outreach care and delivering allied health interventions within their clinical scope of practice to clients who have chronic health conditions and/or complex healthcare needs.
The role involves facilitating collaborative integrated care planning across acute and community based services.
The role of the Community Care PT/ OT provides discipline specific interventions and supports both professionals and service users to navigate the health service system, ensuring responsive, patient-centric outcomes for all.
