People First Healthcare is a registered aged care and NDIS provider located in Victoria Roxburgh and NSW Blacktown. We are passionate about providing high-quality care and support services to vulnerable members of our community, and we are currently seeking an experienced and motivated NDIS Support Coordinator.
About the Role
As an NDIS Support Coordinator, you will be responsible for coordinating and managing the delivery of services to NDIS participants. You will work closely with participants, their families, and other stakeholders to ensure that their individual goals are achieved and that they receive the best possible outcomes.
Key Responsibilities
- Conduct initial assessments with participants to identify their needs and goals
- Develop individualised support plans for participants in collaboration with them and their support network
- Identify and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements
- Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved and services are delivered in accordance with the participant's plan
- Monitor participant progress and make necessary adjustments to their support plan as required
- Maintain accurate and up-to-date records of participant progress and service delivery
- Communicate effectively with participants, their families, service providers, and other stakeholders to ensure that everyone is informed and involved in the participant's support plan
- Participate in ongoing professional development to ensure that knowledge of NDIS policies and procedures is up-to-date
Qualifications
- A relevant tertiary qualification in disability, social work, psychology, or a related field
- Knowledge and understanding of the NDIS and its policies and procedures
- Experience in providing support coordination or case management services
- Excellent communication and interpersonal skills
- Strong organisational and time management skills
- Ability to work independently and as part of a team
- A valid driver's license and access to a reliable vehicle
Working Conditions
This is a full-time position. The role may require some travel to meet with participants and service providers.
