ABOUT THE ROLE
Platinum Support Coordination is looking for someone who has the right attitude and values to join our team and put thier experience & knowledge of support coordination to good use.
This position requires a highly organised person with excellent oral, written and interpersonal communication skills. We are looking for a person who can work independently and efficiently as a member of our team, and someone with a minimum of 2 years experience in the roll.
This role requires you to be in the office for at least 2-4 weeks for training. Following this period a remote work model will be available.
- Support senior coordinators in administrative duties
- Build rapport, negotiate and develop relationships with stakeholders, Local area coordinators (LAC's) planners and service providers to align with client's NDIS goals
- Conduct face to face meetings with clients for initial onboarding/intake process
- Organise services for each individual client including providing a person-centred assessment and care plan
- Plan and review clients funding on a regular basis
- Prepare reports for review as per the NDIS guidelines
- Maintain ongoing communication with clients, carers and families to ensure they are aware of rights and responsibilities to make informed choices
- Assist clients in navigating the NDIS scheme
ABOUT YOU
- Minimum Cert IV qualifications in either Mental Health, Disability or Community Services
- Experience with Psychosocial and Neurological disabilities (advantageous)
- Competent organisational and time management skills with excellent attention to detail
- Strong written and verbal communication skills (bilingual is advantageous)
- Have knowledge or understanding of the NDIS

Salary will be determined in accordance with the SCHADS award and will be determined based on the applicants skill set and experience.
