Our person-centred model of mental health support strives to provide each of our participants with the right support worker to empower them to build capacity and achieve their NDIS goals.

About the Role

The role involves overseeing and supporting a team of key workers and participants. The primary focus is to ensure that the services provided meet both organisational standards and the needs of the participants.

Key responsibilities will include:

  • Building and maintaining strong participant relationships and support networks.
  • Conducting monthly 1:1 meetings with our field staff.
  • Collaborating with the Rostering Officer to resolve shift coverages and providing guidance on service delivery and scheduling.
  • Monitoring our Client Management System, ensuring completion of shifts, case notes and processing weekly timesheets.
  • Monitoring complaints and feedback system.
  • Acting as a communication support point for participants during crises and connecting them to relevant support services.
