About the opportunity

Primary Health Tasmania now has an exciting opportunity for an innovative and proactive Lead - Integrated Health Care to work in the areas of Aged Care and Palliative Care developing and implementing priority integrated health service and system improvement strategies and projects.

What would you be doing

Working closely with the Manager Sector Improvement, you will apply your sector knowledge and take the lead in coordination and integration of a variety of sector improvement and health program activities – mainly focusing on the program areas of Aged Care Regional Planning on aged care reforms and Greater Choice for At Home Palliative Care Measure. You will work collaboratively across teams to identify and address priorities for improving integrated health capabilities in the primary health workforce and with service providers.

Here’s what you’ll look like

  • Demonstrated experience in leading and developing priority projects within a health sector environment.
  • Subject matter expertise in aged care or palliative care or a similar field would be highly regarded
  • Experience in developing and implementing whole of system frameworks and regional planning
  • Advanced communication, negotiation and problem solving skills and the ability to work collaboratively with consumers, communities, service providers and partners to influence positive change
  • Proven success in leading teams to deliver high quality results
