Primary Health Tasmania now has an exciting opportunity for an innovative and proactive Lead - Integrated Health Care to work in the areas of Aged Care and Palliative Care developing and implementing priority integrated health service and system improvement strategies and projects.
Working closely with the Manager Sector Improvement, you will apply your sector knowledge and take the lead in coordination and integration of a variety of sector improvement and health program activities – mainly focusing on the program areas of Aged Care Regional Planning on aged care reforms and Greater Choice for At Home Palliative Care Measure. You will work collaboratively across teams to identify and address priorities for improving integrated health capabilities in the primary health workforce and with service providers.
