- Homeless to Home Support Worker
- 12 months fixed term -Part Time - 19 hours per week
Role purpose
The role of Support Services is provision of specialist homelessness services and other health and human services (through a multidisciplinary integrated model) H2H community partner will provide and support clients with support packages in their transition from emergency accommodation into medium term housing and beyond, consistent with need.
Case management and wrap around services will focus on the physical, emotional and cultural safety and wellbeing of clients. Support may include ensuring access to counselling, crisis resolution, personal care and life skills, family and parenting support and liaison with education or employment services for the client and affected family members. The Support services provider must deliver persistent, responsive, targeted and integrated case managed interactions for clients through flexible contact hours sufficient to create engagement and build trust and address their immediate homelessness and longer-term housing stability, however establishing a reasonable minimum threshold of contact hours spread over a seven-day week period depending on client’s needs.
About You
- Have demonstrated knowledge of the local Aboriginal and Torres Strait Islander community and culture and understanding of factors that impact he social, economic and spiritual wellbeing of Aboriginal families.
- Well-developed interpersonal and communication skills, both verbal and written, including the ability to engage with consumers with complex socioeconomic needs.
- Demonstrated ability to identify and priorities needs, provide people centred support and assistance, including access to integrated services for community experiencing gambling harms or presenting with complex needs.
