As a Level 3 NDIS Support Coordinator, you will play a crucial role in assisting individuals with disabilities to navigate the National Disability Insurance Scheme (NDIS) and access the supports they need to live independently and achieve their goals.
You will work closely with clients, their families, and service providers to develop and implement comprehensive support plans tailored to each individual's needs.
Key Responsibilities:
- Conduct comprehensive assessments of clients' needs, goals, and aspirations.
- Develop individualised support plans in collaboration with clients, focusing on maximising their independence and quality of life.
- Assist clients in understanding their NDIS plans and funding, providing guidance on available supports and services.
- Liaise with NDIS representatives, service providers, and other stakeholders to coordinate the delivery of supports outlined in clients' plans.
- Monitor and review support plans regularly to ensure they remain aligned with clients' evolving needs and goals.
- Advocate on behalf of clients to ensure they receive the appropriate supports and services outlined in their NDIS plans.
- Provide guidance and support to clients in resolving issues or concerns related to their NDIS funding or service provision.
Requirements:
- Minimum of 2 years' experience working in disability support, preferably in an NDIS-related role.
- Strong understanding of the NDIS framework, including eligibility criteria, funding categories, and support coordination guidelines.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish positive relationships with clients and stakeholders.
- Demonstrated ability to assess individual needs, develop support plans, and coordinate services to meet those needs effectively.
- Ability to work independently, autonomously and collaboratively as part of a multidisciplinary team.
- Knowledge of relevant legislation, policies, and procedures related to disability services in Queensland.
Knowledge of relevant legislation, policies, and procedures related to disability services in New South Wales AND experience working with incarcerated or previously incarcerated individuals would be considered as highly advantageous for this role.
