We are seeking a compassionate and organised Mental Health Client Care Coordinator to join our team and assist in implementing care plans developed by our multidisciplinary team.

Position Overview:

This role will primarily be work from home, with occasional in-person client engagement. The Mental Health Client Care Coordinator will be responsible for ensuring that care plans are implemented effectively, ensuring staff receive comprehensive handovers, and providing the necessary education and support to optimise client outcomes. You’ll be working closely with clients, their families, and healthcare professionals to ensure they receive the best possible care.

Key Responsibilities:

  • Implement care plans developed by the multidisciplinary team, ensuring services are delivered according to each client’s needs.
  • Coordinate and manage the day-to-day care of clients, ensuring comprehensive, person-centred support.
  • Ensure staff members receive detailed and effective handovers to maintain high-quality care.
  • Provide ongoing education and training to staff, ensuring they have the knowledge and resources to achieve client outcomes.
  • Monitor and assess client progress, making adjustments to care delivery when necessary.
  • Serve as the primary point of contact for clients, their families, healthcare professionals, and community services.
  • Provide emotional support and crisis intervention when required, advocating for clients’ needs.
  • Maintain accurate and up-to-date client records in compliance with confidentiality and data protection requirements.
  • Participate in team meetings, contributing feedback on client progress and assisting in improvements to care delivery.
  • Occasional client engagement in person when necessary.

Qualifications:

  • Relevant qualifications in mental health, social work, psychology, or a related field.
  • Previous experience in client care coordination or a similar role, preferably in the mental health or social services sector.
  • Strong understanding of mental health conditions and the recovery model.
  • Excellent organisational, communication, and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Experience in providing education and training to staff.
  • Empathy and a commitment to supporting individuals with mental health challenges.
  • Knowledge of local community resources and services is an advantage.
  • A valid driver’s licence and access to a reliable vehicle may be required for in-person client engagement.
