Position Overview:

As a Mental Health Case Manager & Shift Coordinator, you will manage a caseload of clients, conduct risk assessments, coordinate team shifts, and actively contribute to care team meetings. This role focuses on case management, team collaboration, and ensuring clients receive the necessary care and resources for their recovery journey—all from the comfort of your own home.

Key Responsibilities:

• Manage a caseload of clients, providing case management and monitoring progress remotely.
• Coordinate shifts and schedules for team members to ensure effective coverage and service delivery.
• Attend regular virtual care team meetings to discuss client progress, adjust care plans, and address any barriers to recovery.
• Conduct risk assessments for each client to identify potential risks, vulnerabilities, and needs, and develop strategies for risk mitigation.
• Develop and maintain individualised care plans in collaboration with clients and other service providers.
• Serve as a liaison between clients, service providers, and community resources to ensure comprehensive, coordinated care.
• Provide crisis management and support as needed, offering immediate assistance to clients and team members.
• Regularly adjust care plans based on feedback and progress from clients and the care team.

Qualifications:

• At least 2 years of experience in mental health field.
• Strong experience in conducting risk assessments and developing risk management strategies.
• Excellent organisational and multitasking abilities, particularly in coordinating remote schedules and managing caseloads.
• Strong communication skills and the ability to collaborate effectively with clients and team members.
• Experience with telehealth or remote case management preferred, but not required.
• A passion for supporting individuals through their mental health recovery journey.
