We are looking for an experienced Support Coordinator to join our team in a casual role.
You will have the flexibility to work from home.
You will be:
- Responsible for planning, developing and coordinating services
- Responsible for interpreting participants’ NDIS plan,
- Assisting participants in the planning and implementation of services
- Helping participants gain skills to allow them to become independent in managing their own supports
- Monitoring and reviewing plans and goals in consultation with participants on an ongoing basis.
- Arrange any necessary assessments that participants may require.
ABOUT YOU:
- Have an enthusiastic and compassionate nature
- Excellent communication and interpersonal skills
- Have experience as a Support Coordinator or a similar role working within the NDIS setting
- Ability to liaise and build rapport with participants, families and support workers.
- Able to show knowledge of NDIS and its framework
- Experience with both mental health and disability participants
- Exceptional time management and organisational skills with the ability to priorities competing demands.
- Hold NDIS Screening Check
- Currently hold or willing to obtain a National Police Check and Working with Children Check
- A valid driving license and access to a vehicle is required
