Nurse Practitioner - Aged Care / Palliative Care

We are seeking a dynamic Nurse Practitioner to join our Clinical Services team in a Full Time Perm capacity, with a base at our Head Office in Wayville the successful applicant will move between our residential homes and community service locations. You will be responsible for:

  • Consulting with Multi-disciplinary teams in order to promote optimal resident health outcomes, including  palliative care
  • Function autonomously and collaboratively in an advanced and extended clinical role with other health professionals using high level clinical knowledge
  • Maintain compliance to professional standard of practice in accordance with the Australian Health Professionals Registration Authority Competency Standards and legal and ethical requirements
  • Ensuring effective and efficient management of aged care residents and clients through optimal resident/client care outcomes within the aged care and palliative care scope of practice.
  • Initiating and managing the care and/or monitoring the ongoing therapy of clients or residents with individual care needs.

To be successful in your application your skills will include:

  • Registered as a General Nurse and endorsed as a Nurse Practitioner with AHPRA is essential
  • At least 5 years of clinical experience
  • Demonstrated ability to work as an autonomous practitioner
  • Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation
  • Demonstrated advanced level of evidence based nursing practice in the care and management of older people and people requiring palliative care
  • Knowledge of continuous improvement principles as applied to aged care
  • Demonstrated ability to participate/initiate and or facilitate high quality research or clinical change.
  • Current Driving Licence and ability to travel across sites is essential 
  • Meeting current Influenza and Covid Vaccination requirements
