The Case Worker – Youth Justice is responsible for the delivery of the Hope Agreement. You will be an integral part in the collaboration with other consortium partners by attending meetings, sharing good practice, collating data, reporting upon outcomes and promoting the service.
In this role you will manage a case load of between 5 – 8 young people, dependent on case complexity. You will help young people to address complex issues that are negatively impacting their life, skills, health and development. This includes, mentoring & supporting young people to access health services such as registering for a GP, dentist or psychologist, facilitating referrals to other services and increasing leisure and recreational opportunities.
This role is part-time, working 19 hours per week across 3 days, working days can be negotiated, however, they must be on business days (no weekends). You will be required to travel to various metro locations across Perth to meet clients, transport clients & liaise with external agencies in addition to attending various Rise offices, with some working from home. A work vehicle will be available for use when you travel to meet clients & external agencies.

Appointments to this position will be subject to a NDIS Worker Screening Check / NDIS Worker Orientation Module / Working with Children Check / Department of Justice Clearance / current driver’s licence.
Costs for new checks will be covered and/or reimbursed by Rise.
