Are you an experienced NDIS Support Coordinator looking for a rewarding role in a supportive and flexible work environment? Rise Wellbeing is seeking a compassionate and skilled professional to join our team initially on a casual basis, with opportunity to expand to permanent part or full time after 6 months.

Rise Wellbeing is a dedicated disability support practice committed to empowering individuals and exceeding client expectations. We provide a person-centred, strengths-based approach to NDIS participants, ensuring they receive the best possible support to achieve their goals.

The Role

As a Support Coordinator, you will:

  • Assist NDIS participants in understanding and implementing their NDIS plans.
  • Connect participants with services that align with their needs and goals.
  • Support individuals to build capacity and navigate the NDIS system confidently.
  • Maintain accurate case notes and reports.
  • Work collaboratively within a caring and professional team.

About You

  • Minimum 1-2 years’ experience as an NDIS Support Coordinator.
  • Relevant tertiary qualifications in Social Work, Psychology, Counselling, Community Services, Disability Support, or a related field.
  • Strong understanding of the NDIS, its processes, and service providers.
  • Excellent communication, organisational, and problem-solving skills.
  • Ability to work autonomously while contributing to a team environment.
  • Passionate about making a positive impact on people's lives.
  • NDIS Worker Screening Check, working with children & current police check (or willingness to obtain).
  • Current driver’s licence and access to a reliable vehicle.
