About the role
The Supports Coordinator is a primary point of contact for participants receiving Supports Coordination services through Rural Lifestyle Options Australia. They are responsible for developing and maintaining a network of service providers and working collaboratively and innovatively with the NDIS and service providers to ensure services and supports offered meet the needs of the Participant. The Supports Coordinator works proactively with Participants and their families and/or carers to build capacity and knowledge when coordinating NDIS plans.
The Supports Coordinator is required to travel to meet with NDIS participants and key stakeholders within the region.
This role will work collaboratively with RLOA's NDIS Intake and Support Officers for South East Queensland and Northern Rivers and reports to the Area Manager (GC and NSW).
Key responsibilities include:
- Proactively building and maintaining positive collaborative stakeholder relationships with other agencies, supports coordinators, plan managers, LAC's and NDIS planners;
- Have a thorough practical working knowledge and understanding of person-centred approaches to service offerings/supports;
- Understands a person’s goals and requirements and their individual support plan in circumstances where complex care and/or significant environmental factors exist;
- Working within the principles of the National Disability Insurance Scheme and relevant legislation;
- Organises referral through a comprehensive understanding of the range of support and services offered;
- Providing reports as required to the Area Manager (GC and NSW);
- Participating in meetings as required by the role;
- Ensuring all documentation is completed to meet relevant standards and audit requirements.
About you
- Equivalent to VET Diploma/Advanced Diploma/Associate Degree, or equivalent knowledge and experience obtained through ongoing professional development.
- Willingness and ability to drive innovation and problem solve in an ambiguous environment;
- A thorough understanding of NDIS service offerings and guidelines, and the services offered by Rural Lifestyle Options Australia or the ability to rapidly acquire;
- Demonstrated experience in working autonomously;
- Current open drivers licence;
- Excellent customer service skills and strong communication skills with the ability to tailor communication to meet the needs of key stakeholders;
- Accurate report writing skills.
