We are seeking an individual to join our team based in our ANZAC Village in Sydney’s stunning Northern Beaches. As a Case Manager for our Homes for Heroes program, you will be a primary point of contact for intake and referrals, you will collaborate closely with the Veteran Housing Program Manager and those in peer positions in providing front facing critical case management support to veterans who are experiencing homelessness or at risk of becoming homeless.

This role is instrumental in advancing Homes for Heroes vision, empowering veterans to break the cycle of homelessness. Your focus will be on supporting each Veteran with long-term housing goals by working closely with them and a range of providers to create a Transitional Housing Support Plan with clear objectives and goals to achieve this outcome.

Please note this is a 12 month fixed term contract role with a possibility of moving to a permanent role.

A bit about you: 

  • Relevant qualifications in health, welfare, community services, or a mental health discipline
  • Experience in case management- in a front facing role- supporting individuals with history of homelessness and with complex needs and supporting individuals with a trauma-informed, recovery-oriented approach
  • Minimum 5 years’ experience in client focused roles
  • Knowledge of the Australian Defence, ESO, and Veteran communities, with the ability to navigate relevant agencies and departments
  • Experience in supporting individuals within the homelessness sector and experience in finding housing solutions
  • Demonstrated ability to create and implement Support Plans based on SMART goals
  • Proven ability to work under pressure, handle difficult situations, and manage a diverse caseload effectively
  • Willingness to work occasional weekends or evenings and attend work related events
